The new Health Benefit Card vendor, RT Consulting LLC, will visit the Osage Nation Fairfax Family Service Center at 401 S 8th Street, Fairfax, OK 74637
Wednesday, October 16, 10 a.m. - noon
If you are interested in enrolling or need assistance enrolling, please stop by with a photo ID and your Osage Nation Membership Card. Both paper and electronic enrollment will be available and a representative will be available to answer questions about the program. *NOTE*
These enrollment opportunities DO NOT involve our Plan F/Part D benefit that is administered by another vendor. This is not only for the Health Benefit Card program that offers $500 a year for members 64 & under, and $1,000 a year for members 65 & older.
