Osage Nation Communities of Excellence (COE) has received a small grant from the Oklahoma City Area Inter-Trial Health Board (OKCAITHB). The Osage Nation is a board member tribe among approximately 43 tribes. OKCAITHB is a non-profit organization whose directive is “uniting for one cause … Strong Indian people making healthy choices throughout their circle of life.”
This Community Health Survey is for Osages living in Osage County and will provide a way to get a better understanding of what is available to Osage people in the county. It is a quick, reader-friendly survey which will only take a few minutes to complete. One does not have to answer all the questions and may opt out at any time.
The information ranges from healthy food availability in your community to exercise opportunities. There is also a section on tobacco cessation. COE will be conducting more surveys throughout the year, and appreciates your help.
