The new Health Benefit Card vendor, RT Consulting LLC, will visit the Osage Nation Title VI at 350 Senior Drive, Pawhuska, OK 74056
If you are interested in enrolling or need assistance enrolling, please stop by with a photo ID and your Osage Nation Membership Card. Both paper and electronic enrollment will be available and a representative will be available to answer questions about the program. These enrollment opportunities DO NOT involve our Plan F/Part D benefit that is administered by another vendor. This is not only for the Health Benefit Card program that offers $500 a year for members 64 & under, and $1,000 a year for members 65 & older.
