To amend fiscal year 2009 appropriation Act ONCA 08-43 and fiscal year 2011 appropriation Act ONCA 10-97 for the Diabetes Program; to amend fiscal year 2010 appropriation Act ONCA 10-25 to appropriate twenty six thousand eight hundred forty four dollars ($26,844) of federal funds to Community Health Representative and to amend fiscal year 2012 appropriation Act ONCA 11-39 to appropriate seven thousand seven hundred fifteen dollars ($7,715) in tribal funds and two thousand seven hundred thirty two dollars ($2,732) in federal funds to Community Health Representative; and to establish an alternate effective date.
